This is an interactive form script to receive your users' comments, feedbacks, suggestions or information. It can be used to create as many forms as possible with different form fields such as file upload, text box, check box, radio button, selection list etc. Those fields can be defined to suit different requirements. For a consistent style in your site, you can design your layout by your web page authoring tools. Once your users submit the form, the data is stored in MySQL database. It includes a web-based control panel to view, reply or delete your users' messages. It has an option to set whether you would receive email notification after someone has submited the form.
